Your Community Health is proud to be the lead agency of a project in partnership with Ballarat Community Health, Austin Health and Thorne Harbour Health.
The Victorian Government has funded an initiative to increase services for trans, gender diverse and non-binary communities and we are establishing a new multi-disciplinary clinic in Preston. Ballarat Community Health will also be operating a new multi-disciplinary clinic in Ballarat to improve service access for regional community members.
We aim to provide gender affirmative health care services and it all starts with making an appointment with one of our Peer Navigators. A Peer Navigator is a person of lived experience who also has the professional skills to support you to create pathways to affirm your gender and access health services depending on your personal needs and goals.
Whatever your goals are, whether it be to socially, medically or legally affirm your gender or simply just to seek a health care provider who can provide general health services, give our Peer Navigator a call to make an appointment.
The clinics will provide services including:
- General medical services
- Hormone management and support
- Sexual health services
- Counselling and mental health support
- Health and well being programs, and more
