Victorian Trans and Gender Diverse Community Health Services
The Victorian Government has funded an initiative to increase health services across Victoria for trans, gender diverse and non-binary people aged 18 and over. Your Community Health is the lead agency of this project, in partnership with Ballarat Community Health, Austin Health and Thorne Harbour Health.
We aim to provide gender affirmative health care services and it all starts with making an appointment with one of our Peer Navigators. A Peer Navigator is a person of lived experience who also has the professional skills to support you to create pathways to affirm your gender and access health services depending on your personal needs, goals and where you are located. Our Peer Navigators are able to support all trans, gender diverse and non-binary Victorians with information, support and referral to health services.
Multi-disciplinary clinics have been established in Preston with Your Community Health and Ballarat with Ballarat Community Health with specialist support from Austin Health. A number of our services can be accessed via telehealth or by phone, so if you can’t get to one of our clinics in person we are still able to help you by either providing services online, by phone or by referring you to a health service in your local area.
