Diabetes Education

Your Community Health supports people with diabetes to make important lifestyle changes that will help them look after their diabetes and reduce their risk of diabetes-related complications.

We offer both individual sessions to help support your specific needs, and group sessions for people wanting to connect with others with Type 1 or Type 2 diabetes.

Diabetes Education

Diabetes Educators are health professionals who have undertaken further education in diabetes management and health care. They work with people who have diabetes to increase knowledge and skills in how to manage diabetes and support people in the changes they would like to make to live a healthy life. They work with you and your GP to help you to manage diabetes if your diabetes control changes, or becomes difficult.

Who: Adults with Type 1 and Type 2 Diabetes

When: Mondays, Wednesdays and Fridays

Where: East Reservoir, Northcote and PANCH (Preston) Health Centres

Cost: Fees may apply depending on your circumstances.

Booking: Booking is essential. Call (03) 8470 1111.

 

Annual Diabetes Cycle of Care Review

It is important to have annual checks to help identify, monitor and prevent the complications of diabetes.

This comprehensive annual review involves an appointment with a Diabetes Educator. hey will review the management of your diabetes, answer your questions and help you to access a Podiatrist, Dietitian and Optometrist at Your Community Health. A written report will be sent to your Doctor following the appointment.

Who: Adults with Type 2 Diabetes

When: Mondays, Wednesdays and Fridays

Where: East Reservoir, Northcote and PANCH (Preston) Health Centres

Cost: Fees may apply depending on your circumstances.

Booking:  Booking is essential. Call (03) 8470 1111.

 

Darebin Diabetes Support Group

This peersupport group is for people wanting to learn more about diabetes and share their experiences. Guest speakers regularly attend and present to the group.

Who: Adults with Type 1 or Type 2 Diabetes.

When: First Monday of every month

Where: East Reservoir Community Hub

Cost: $3 per session

Booking: No booking is necessary

 

HEAL (Healthy Eating, Activity and Lifestyle) Program

The HEAL program helps people to develop lifelong healthy lifestyle habits. The weekly 2 hour group session includes 1 hour of physical activity and 1 hour of lifestyle education. The program lasts for 8 weeks.

Who: People with diabetes, pre-diabetes, chronic illness or people wanting healthy weight management.

Where: Your Community Health Northcote Health Centre
